COVID-19 SAFETY PRECAUTIONS
MASKS
We have worked tirelessly to source USA lab tested KN95s and N95s for not only our team members, but for all of our guests as well. Upon arrival, you will be greeted outside of the salon and given a KN95 mask to mold to yourself. While we understand that it can be constricting to wear a KN95 for extended periods of time, we feel that this is the only way to ensure everyone’s safety. No one (including our team members) will be allowed to remove their mask and “free breath” inside of our space. You also may not opt to wear your own non-KN95 mask, nor wear gloves that have been previously worn. Mask breaks will be permitted outside only. This is our methodology for mimicking a hospital isolation suite as closely as possible. Combined with our cleaning procedures, this will heavily eliminate the risk of disease spread.
Cleaning Methods
In addition to wearing N95 masks, all of our team members will be covered by face shields and disposable gloves. Our cleaning and sterilization products are 70% + alcohol solutions, hydrogen peroxide solutions, and surgical grade sani-wipes. Our full team has been trained on when and where to use these various products and how to apply them properly to ensure the highest degree of cleanliness. All occupied areas and surfaces (i.e. styling stations and shampoo sinks) will be sanitized between client usage. All styling tools will also be sanitized between each client using Barbicide, which is 10x stronger than Lysol, and has been an industry standard for 75 years.
Waiver
PLEASE REVIEW THE FOLLOWING WAIVER WHICH YOU WILL BE REQUIRED TO SIGN PRIOR TO YOUR APPOINTMENT:
Due to the global pandemic outbreak of COVID-19 and for the safety and protection of the health of our staff, stylists, and guests, we are implementing additional safety measures in compliance with the safety precautions published by the Center for Disease Control (CDC) and the New York State Department of Health.
I confirm that I am not presenting with any of the following symptoms of COVID-19 identified by the CDC:
Fever > 100F
Chills
Cough
Sore Throat
Shortness of Breath
Flu-Like Symptoms
Loss of Smell or Taste
All stylists and all employees of Laura Braunstein LLC dba Laura Braunstein Hair Studio agree to abide by these same standards and affirm the same. We also affirm that we have expanded our sanitation protocols to more thoroughly prevent the spread of COVID-19 and other communicable conditions. I understand that I must immediately sanitize my hands upon entering the salon and that I must wear a mask that covers my mouth and nose at all times. I understand that I cannot remove my mask during my visit.
Only guests receiving a service will be allowed into the salon due to distancing and capacity guidelines. I understand I may not bring children nor anyone else who does not have an appointment with me into the salon.
I verify that I have not returned to New York from any country outside of the United States, whether by car, air, bus or train within the past 14 days.
I understand that physical distancing of 6 feet may not be possible while in the salon receiving services. Our team will maintain six (6) feet of distance between themselves and you, our guest, as much as possible in the salon setting. We will also be requiring this same procedure for guest-to-guest contact in order to limit the exposure to all individuals.
I understand that Laura Braunstein LLC dba Laura Braunstein Hair Studio stylists, staff, and all others will not be held liable for any exposure to COVID-19 or any other contagion.
I verify that the information that I have provided on this form is truthful and accurate. I understand all of the potential risks and complications related to COVID-19 and I would like to proceed with my desired salon services.
WAIVER AND RELEASE, I HEREBY WAIVE ALL CLAIMS AND FOREVER RELEASE Laura Braunstein LLC dba Laura Braunstein Hair Studio including its officers, directors, employees, successors, assigns, agents, heirs, representatives, and volunteers as applicable, in their individual or corporate capacities, of all claims, liabilities, agreements, and causes of action of any nature due to any injury, loss, or damage to person or property, that may arise out of my services performed at Laura Braunstein Hair Studio .
By signing below, I agree to the above statements and release Laura Braunstein LLC dba Laura Braunstein Hair Studio its staff and practitioners from any and all liability for any unintentional exposure or harm due to COVID-19.
